At Allied Medical & Rehabilitation, PC our initial examination procedures are geared toward the detection of the cause of your condition. It is our goal not to focus on your pain, but the cause of your pain. By determining the cause of your condition, we can decide if we can help you, and how best to treat you. This begins by having a thorough consultation with one of our doctors. During the consultation we will learn about your past medical history and your present condition. This is followed by a physical examination that will be tailored to your problem.

Our doctors take the time to listen. Please be leave adequate time to have all of your questions answered.

We urge you to write down your questions prior to your visit, so that no question goes unanswered.

Following your initial visit you will be scheduled for your "Report of Findings", this is where we will review your examination results. We will explain why you are suffering with your condition and let you know how we can help you. We will discuss your "Care Plan" and go over any special instructions you need to in order to obtain the fastest results.

 
What to Bring to Your First Appointment:
  • A list of current prescription and over-the-counter medications with the dose and frequency
  • Your insurance card
  • Physician referral forms if required by your insurance provider
  • Pertinent information about your medical and surgical history
  • Police or accident report if applicable.
